Waiver of Liability *	I agree to the terms and conditionsUnderstanding the voluntary nature of my participation in the work, programs, and activities offered through Natural Dharma Fellowship (NDF) , and in return for the benefits I receive from them, I release and discharge NDF and its agents and employees, and I promise to hold NDF their agents and employees harmless, from all causes of action, liability and claims for all personal injuries, illness, property damage or death which I may now or hereafter have, which are caused by, or are directly or indirectly related to, any work I may perform or any activities in which I engage during my participation in this program or my stay at the center. For the period of this retreat, by checking this box, I agree that I am covered for my medical insurance and these areas are not the responsibility of Natural Dharma Fellowship. I agree to accept full responsibility for the security of my personal belongings. Wonderwell is not responsible for lost, missing, or stolen items.

Do you have any problem stepping into a tub shower?	Yes No Several of our rooms have tub showers. While we cannot guarantee availability of rooms with step-in showers, if you have any problem stepping into a tub shower, we will do our best to place you accordingly.
Comments or Special Needs	Please let us know how we may support you regarding any severe non-dietary allergies, physical limitations, hearing problems, environmental intolerances, late arrival, early departure, or other special needs.
Program Participation Agreement & Release *	I agree to these terms and conditions While in retreat here at Wonderwell, we ask all guests to strictly adhere to the 5 precepts during a retreat program: 1) refraining from lying or gossiping, 2) refrain from taking what is not given, 3) no sexual misconduct, 4) not causing harm to others, and 5) no consumption of drugs or alcohol. Violation of these precepts will be grounds for dismissal. If you are having difficulties that may lead you in one of these directions, please contact a Teacher or the Retreat Manager. Need to Self-Monitor Food Allergies: I understand that I am responsible for knowing whether any ingredients to which I am allergic exist in any foods that are prepared by Wonderwell. If I am uncertain about the ingredients of these foods, I am responsible for obtaining that information from the cooks who have prepared or overseen the preparation of such food. Social media postings and photos by other retreatants: NDF recognizes that a portion of its guests may wish to photograph, video, and audio record aspects of their program experience to post on various social media sites. In order to protect the privacy of others, NDF requires that all guests agree not to post or publish any images, video, or audio recordings of other individuals, or make use of the Natural Dharma Fellowship or Wonderwell name or logo, without express permission. I understand that I am solely responsible and potentially liable for any harm or damages caused by my online activity relating to Natural Dharma Fellowship or Wonderwell Mountain Refuge. Release of Liability: After being informed of the above risks and responsibilities, I generally release Wonderwell Mountain Refuge and Natural Dharma Fellowship, its teachers, instructors, agents and other representatives, from all claims, causes of action, medical expenses, and other costs related to my guest participation, whether online, or from my later use of information or instruction at home. I will abide by the terms of the Participation Agreement and Release because it makes me aware of NDF’s policies and creates a legally binding general release of liability.
